You put side load on the caster wheels which they were not designed for by re-mounting them as fixed wheels.
Entirely your own fault for using a component in an application it was not designed for!

Tap Type?
Are you tapping the bore of Carbon Fibre tube?
If so why, and have you considered the impact of that on it's structural integrity?

Go ER40 instead, it's far less limiting and contrary to the opinion of those who haven't used one you can turn square, hex, octagonal and tapered stock in ER40 collets
