I have several of those bars that use triangle inserts. I've always used them on the mill. Mine are all short.

Would you start off with a small HSS lathe blank? Maybe a broken drill bit? Hmmm. Perhaps a piece of triangular file?

Here is the bar and insert I used. I think the insert is just a piece of HSS. If so, how do I replace it? I do not have any way to grind such a small piece.
