I have seen a steam donkey setup with a tangent burner and a firebrick lined lower ash pan that the fire swirled in as it makes itself around the firebox before going up the flues, given a longer combustion path.

What you need to look at are carter bros style trucks. They have the extended bars to the outside where the rigging hung from. Maybe Curtis Ferrington will chime in, he has done a lot of research on carter bros trucks. If you have the West Side Pictoral by Mallory you could see the examples.
