Recent Amtrak/CSX tradegy in South Carolina

On CBS 60 Minutes the conductor was lamenting his firing by CSX. The engineer was fired too.
The conductor said it was the responsibility of the engineer to re-align the switch and he was
assured this was done. Aside from PTC shouldn't this have been prevented somehow?

In a recent blurb on TV the engineer said it was the duty of the conductor to realign the switch. I think he is right. Actually, it should be both. One should be checking what the other does to avoid incidents like this. But as the engineer said, "it is the responsibility of the conductor to align the switch. If he had to get down off the engine every time they had to work a switch nothing would get done."
